SuperSenses assessment kit delivery address

Where should we ship the SuperSenses assessment materials? Smell-training materials are sourced separately from the SuperSenses assessment kit.

Who will own and use SuperSenses at your clinic?

Start with the implementation owner, then add anyone who will administer the assessment, recommend it to participants, manage care, or handle billing. Check every role each person covers.

Roles
Step 2 of 6 · Launch team

How many people at your practice will use SuperSenses?

Where should we send invoices?

Add the accounts payable contact or AP alias and choose how invoices should be delivered.

Invoice delivery

Launch session

A 60-minute working session prepares the team to launch: account access, the participant journey, assessment setup, results, and follow-up. We ask now so the right people attend.

Certification

Certification confirms that people who administer the assessment can set it up consistently and explain the participant experience. It is completed during the launch session.

Step 3 of 6 · Assessment

How will participants take the assessment?

This sets the participant journey first, so the training, materials, and follow-up choices that come next match how the assessment will actually be delivered.

Step 4 of 6 · Smell training

How will smell-training materials be made available?

Choose the path the app should recommend after a qualifying result: the participant can source materials directly, or your clinic can source and provide them from your office.

Participant base experience

The app starts with four familiar things from home. Participants can also search the web for available smell-training kits or say they already have one.

2+ senses

Dual-impairment follow-up

When more than one sense needs follow-up, give the participant one clear destination. The standard approach is primary care review. Choose a different single provider or your organization’s coordination route when that better fits your care model.
